Addison Respite Care Home (ARCH):
Compassionate End-of-Life Hospice Care at Helen Porter Healthcare and Rehabilitation Center in Addison County, Vermont

What is ARCH?
A home for end-of-life care providing compassion, support and comfort care.

Why ARCH?
A need for a peaceful home-like setting for those Addison County Vermont residents unable to spend their final days in their own home.

The last room on the right, Otter Creek Way in Helen Porter Healthcare and Rehabilitation Center, Middlebury, VT.

What is Hospice Care?
A program of palliative and supportive care services providing physical, psychological, social and spiritual care for dying persons, their families and other loved ones.

Who provides the care?
Compassionate care will be provided by trained providers/staff from Helen Porter Healthcare & Rehabilitation Center, Primary Care Physicians, Addison County Home Health & Hospice Nurses and Hospice Volunteer Services.

How do I get referred?
Your Primary Care Physician will refer you to Hospice Care and the path to the ARCH room at Helen Porter, Middlebury, VT.
